Prior to your journey, please read through this section. We want to ensure that your visit to Medfem Fertility Clinic goes as smoothly as possible.

• Before you meet with a consultant, you must complete our new patient forms. Please follow the instructions on the Welcome Pack Form and ensure you gather all of this information prior to your initial consultation. You also need to complete the Cover Sheet document. You may fax these documents to us on +27 (11) 463 1875, email to info@medfem.co.za, or bring along with you on the day of your consultation. • Get a copy of all your medical records and a list of your medications that you are currently taking. If applicable, bring CDs and diagnostic films from past medical procedures.
• Contact your embassy to obtain the appropriate visa/medical visa. Avoid getting a visitor’s visa that cannot be extended easily.
